Dermal Fillers

Dermal fillers can also be called as "liquid facelifts" because they offer extensive benefits without any surgical procedures and in very less time. They are used to restore the youth back in the skin, it can even make the lips appear plump and enhance the contour's shallowness or even the wrinkles and creases appearing on the face can also be diminished. Even though they can't help with the sagging of facial skin, they surely can provide enhanced volume. These fillers have to be repeated and maintained as they are not a permanent remedy to the skin problems. One can use fillers alongside other treatments for rejuvenation of skin like botulinum toxin (botox) injections, etc

The ideal characteristic of dermal fillers is:
  • Safe
  • Durable
  • Hypoallergenic
  • Painless to inject
  • Inexpensive

These dermal fillers should be consistent and must produce results that are predictable.

Dermal fillers may be of variable types and contents irrespective of which, they are injected onto the dermis. Soon after the procedure the applied area is susceptible to inflammation, in order to reduce the itching or discomfort patients are advised to use cool packs over the affected areas. The content for filler materials vary from animal to non-animal or synthetic fillers, of all the most expensive are the cadaveric derived ones that are used only for burn victims.

Dermal fillers

Polymethylmethacrylate With Bovine Collagen

This dermal filler is used for the nasolabial folds correction, removal of acne scars and furrows on the forehead. It has to be injected deep into the deeper layer and not the subcutis. The results of this treatment are permanent and hence its advisable to administer its dose in several sessions. This product should be refrigerated. artefill is an example of such fillers. It's less painful as it contains lidocaine.

Collagen

Collagen is most important structural component of the skin which provides skin elasticity and suppleness. Hence its inclusion as one of the major components of dermal fillers is essentially natural. The first FDA approved dermal filler is the Bovine collagen (eg.: Zyderm I & II , Zyplast.). Later on a synthesized collagen that was bio engineered (CosmoDerm I, II & CosmoPlast) took its place as it didn't require any allergy test prior to the treatment. Refrigeration is a must to all types of collagen fillers.

Polyalkylimide

These semi-permanent dermal filler is often used for treating depressed scars and nasolabial folds. It's also used to replace the volume of face lost due to aging. It is not suitable to eliminate fine wrinkles on the face. It widely enhances the jaw lines and cheekbones. It is only administered after a local anesthesia. After the injection, there is a slow formation of collagen in and around the treated areas within a span of one month's time. Large amount of fillers could also be injected. They can also be removed later on. HIV affected patients who suffer from facial lip atrophy or facial wasting are the ones who are treated using this mode. Polyalkylimide is biocompatible, requires no need of allergy tests before treatment and is radio transparent. These can be clubbed with the use of hyaluronic acid for removal of the finer wrinkles and lines.

Hyaluronic acid

The most prominent Glycosaminoglycan is the hyaluronic acid as it effectively binds itself to water when injected in the skin. It turns the skin softer and highly volumes it. It hydrates the skin as well. It is good in adhesion, cell growth as well as functions like membrane reception. Unlike the collagen fillers these required no refrigeration and was always easy to use and lasted really long compared to the other types. They also didn't require any skin allergy test before the treatment. Hence these fillers had superior cosmetic effects and was always in demand.

Poly lactic acid

Poly lactic acid is bio-degradable, bio-compatible, and synthetic and a peptide that is immunologically inert. They lead to the production of more collagen due to the stimulation of fibroblasts. Rather than a true dermal filling agent poly lactic acid is a stimulating agent that is always sub-dermally injected. As a result of which dermal neocollagenesis happens. Results are very vague and can only be observed within 4 to 5 weeks of treatment. Hence most patients require treatment sessions multiple number of times. Pre treatment allergy test are not required. It is stored at normal temperature and has to be reconstituted before treatment.
